Bluegrass Lacrosse League Waiver TErms
ACCIDENT WAIVER AND RELEASE OF LIABILITY FORM
I hereby assume all of the risks of participating in L4 Lacrosse, including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective equipment or property owned, maintained or controlled by them, or because of their possible liability without fault.
I certify that I am physically fit and have not been advised to not participate by a qualified medical professional. I certify that there are no health-related reasons or problems which preclude my participation in L4 Lacrosse. I acknowledge that this Accident Waiver and Release of Liability Form will be used by the organizers of L4 Lacrosse in which I may participate and that it will govern my actions and responsibilities in said activities.
In consideration of my application and permitting me to participate in L4 Lacrosse, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows:

I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including my traveling to and from L4 Lacrosse events. THE FOLLOWING ENTITIES OR PERSONS: L4 Lacrosse, LLC, Reid Wesley, and/or their coaches, agents, representatives or volunteers.

I INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned in this paragraph from any and all liabilities or claims made as a result of participation in L4 Lacrosse, whether caused by negligence or otherwise.
I acknowledge that lacrosse and any related training may carry with it the potential for death, serious injury, and personal loss. The risks may include, but are not limited to, those caused by terrain, facilities, temperature, weather, condition of participants, equipment, vehicular traffic, actions of other people including, but not limited to, participants, volunteers, spectators, coaches, and lack of hydration.
I consent and agree that L4 Lacrosse, LLC, and/or their coaches, agents, representatives, or volunteers may take photographs or digital recordings of me as a participant during events and use these in any and all media for training or promotional purposes. I further consent that my identity may be revealed therein or by description text or commentary. I waive any rights, claims or interest and I understand that there will be no financial or other remuneration.
The accident waiver, release of liability and image release shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.
I CERTIFY THAT I HAVE READ THIS DOCUMENT, AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I SIGN IT ON MY OWN FREE WILL.

Lightning and Severe Weather Policy
Lightning and Severe Weather Policy
This policy applies to all programs, leagues, camps, clinics, practices, games, tournaments, and other activities operated by L4 Lacrosse LLC, including the Bluegrass Lacrosse League.
The safety of athletes, coaches, officials, staff members, and spectators is the highest priority of L4 Lacrosse. Weather conditions will be monitored before and throughout all outdoor activities. L4 Lacrosse may delay, suspend, modify, relocate, postpone, or cancel an activity whenever weather conditions create a potential safety risk.
When Play Will Be Suspended
All outdoor activity will be suspended immediately when:

Thunder is heard;
-
Lightning is seen;
-
A lightning-detection system or weather-monitoring service indicates an immediate threat;
-
A severe thunderstorm warning, tornado warning, or other dangerous-weather warning affects the playing location;
-
High winds, hail, heavy rain, flooding, or field conditions create an unsafe environment; or
-
An L4 Lacrosse representative, facility representative, athletic trainer, official, or other qualified authority determines that conditions are unsafe.
If thunder can be heard, lightning is close enough to present a danger. Activities will be suspended even when it is not raining at the field.
Once a suspension is announced, all play, warmups, practices, and other field activities must stop immediately. Players may not remain on the field, use an adjacent field, play catch, or return to the playing area until L4 Lacrosse has officially authorized a return.
Safe Shelter
Players, coaches, officials, staff members, and spectators must leave the field and seek shelter immediately.
Approved shelter includes:
-
A substantial, fully enclosed building with electrical wiring and plumbing; or
-
A fully enclosed, hard-topped vehicle with the windows closed.
If no approved building is immediately available, families should return to their personal vehicles. Players should remain with their parent or guardian whenever possible. Coaches are not responsible for supervising players inside individual family vehicles.
The following locations are not safe lightning shelters:
-
Tents or team canopies;
-
Pavilions or picnic shelters;
-
Dugouts or covered benches;
-
Sheds or small outdoor structures;
-
Porches, overhangs, or carports;
-
Golf carts or other open vehicles;
-
Areas under or near trees;
-
Fences, light poles, goals, bleachers, or other metal objects; and
-
Any open area near the playing field.
No individual should remain outside to watch the storm, retrieve equipment, take photographs, or wait for additional instructions.
Required Waiting Period
Outdoor activity may not resume until at least 30 continuous minutes have passed since the last observed lightning flash or the last sound of thunder.
Each new flash of lightning or sound of thunder restarts the 30-minute waiting period.
The end of rain, appearance of sunlight, or movement of a storm away from the field does not eliminate the required waiting period. Participants must remain in approved shelter until L4 Lacrosse provides an official all-clear.
Severe Thunderstorms, Tornadoes, and High Winds
When a severe thunderstorm warning, tornado warning, or destructive-wind warning affects the facility, all participants and spectators must follow instructions from L4 Lacrosse, facility management, emergency officials, and local authorities.
During a tornado warning, individuals should move to the facility’s designated severe-weather shelter when one is available. If sheltering inside a building, move to a small interior room on the lowest available level, away from windows and exterior doors.
Activities will remain suspended until the applicable warning has expired, the immediate threat has passed, and L4 Lacrosse determines that the facility and playing conditions are safe.
Authority to Suspend and Resume Play
An L4 Lacrosse Site Director or designated L4 Lacrosse representative will monitor conditions and has the authority to delay, suspend, modify, postpone, or cancel an activity.
Game officials, facility management, athletic trainers, emergency personnel, and other qualified authorities may also stop play when they identify an immediate safety concern.
The decision to resume an L4 Lacrosse or Bluegrass Lacrosse League activity will be made by the L4 Lacrosse Site Director or designated representative, in consultation with game officials, facility management, medical personnel, or emergency authorities when appropriate.
Coaches, teams, players, and spectators may not independently resume activity. An official all-clear from L4 Lacrosse is required before anyone returns to the field.
L4 Lacrosse may cancel the remainder of an activity when repeated delays, approaching storms, field conditions, darkness, facility limitations, or scheduling restrictions make a safe return impractical.
Communication
Weather delays, resumptions, schedule changes, and cancellations will be communicated through available league channels, which may include announcements at the facility, email, text message, the L4 Lacrosse website, the L4 Lacrosse app, or social media.
Families should continue monitoring official communication while remaining in approved shelter. A lack of immediate communication does not authorize anyone to leave shelter or return to the field.
Individual Safety Decisions
Parents and guardians may remove their athlete from an activity at any time if they believe conditions are unsafe. An athlete will not be penalized for following a parent or guardian’s reasonable safety decision.
L4 Lacrosse LLC reserves the right to take precautions beyond the minimum requirements of this policy whenever circumstances warrant. No game, practice, camp, clinic, or league activity takes priority over participant safety.
Concussion & Head Injury Policy
L4 Lacrosse Concussion & Head Injury Policy
L4 Lacrosse LLC is committed to providing a safe environment for all athletes participating in L4 Lacrosse programs, leagues, clinics, camps, practices, and events, including the Bluegrass Lacrosse League (BLL) and Women’s Bluegrass Lacrosse League (WBLL).
The health and safety of the athlete will always take priority over participation in a game or practice. When in doubt, sit them out.
Recognition of a Possible Concussion
A concussion is a type of traumatic brain injury that may result from a bump, blow, or jolt to the head or from an impact to the body that causes the head and brain to move rapidly.
An athlete does not need to lose consciousness to have sustained a concussion.
Possible signs or symptoms may include:

Headache or pressure in the head
-
Dizziness or balance problems
-
Nausea or vomiting
-
Blurred or double vision
-
Sensitivity to light or noise
-
Confusion or difficulty concentrating
-
Memory problems
-
Appearing dazed or stunned
-
Moving clumsily or having difficulty with balance
-
Slowed responses
-
Unusual behavior or personality changes
-
Fatigue, sluggishness, or feeling "foggy"
-
Loss of consciousness
Signs and symptoms may appear immediately or develop after the injury.
Removal From Play
If an L4 Lacrosse coach, official, staff member, athletic trainer, or other responsible adult observes or suspects that an athlete may have sustained a concussion or significant head injury, the athlete will be immediately removed from play.
The athlete's helmet, protective equipment, or ability to continue playing does not determine whether the athlete is safe to return.
L4 Lacrosse personnel will not attempt to "play through" a suspected concussion.
No Same-Day Return to Play
An athlete removed from participation because of a suspected concussion will not return to participation that day unless evaluated by an appropriately qualified healthcare professional who determines that a concussion did not occur and clears the athlete to return.
If an appropriate healthcare professional is not available at the facility to conduct an evaluation, the athlete will remain out for the remainder of the day's participation.
A parent or guardian's permission alone does not override this policy.
Parent or Guardian Notification
Whenever reasonably possible, the athlete's parent or legal guardian will be notified promptly when an athlete is removed because of a suspected concussion or significant head injury.
The athlete should be evaluated by an appropriate healthcare professional.
L4 Lacrosse coaches and staff members are not responsible for diagnosing a concussion.
Emergency Warning Signs
Some head injuries require immediate emergency medical attention.
Call 911 or seek emergency medical care immediately if an athlete experiences symptoms such as:

Loss of consciousness or inability to wake
-
Seizure or convulsions
-
Repeated vomiting
-
A worsening or severe headache
-
Increasing confusion, agitation, or unusual behavior
-
Slurred speech
-
Weakness, numbness, or decreased coordination
-
One pupil appearing larger than the other
-
Significant deterioration in the athlete's condition
If a serious head, neck, or spinal injury is suspected, the athlete should not be unnecessarily moved until appropriate medical personnel arrive.
Return to Participation
An athlete diagnosed with a concussion may not return to L4 Lacrosse practices, games, training, or other athletic participation until the athlete has received appropriate medical clearance.
L4 Lacrosse may require written clearance from a physician or other appropriately qualified healthcare provider before allowing the athlete to return.
Return to sport should follow the progression recommended by the athlete's healthcare provider. Athletes should not skip steps or accelerate their return solely because an upcoming game, tournament, playoff, or championship is approaching.
If concussion symptoms return during the return-to-play process, participation should stop and the athlete should follow the guidance of their healthcare provider.
Coaches and Officials
L4 Lacrosse coaches and officials are expected to place athlete safety ahead of competition.
No coach, official, parent, teammate, or other individual may pressure an athlete to continue participating after the athlete has been removed under this policy.
The decision to remove an athlete because of a suspected concussion may be made conservatively. L4 Lacrosse personnel do not need proof that a concussion occurred before removing an athlete from play.
Athlete and Parent Responsibility
Athletes are expected to immediately report concussion symptoms or possible head injuries to a coach, official, parent, or L4 Lacrosse staff member.
Parents and guardians are responsible for informing L4 Lacrosse of any medical restrictions or return-to-play instructions provided by the athlete's healthcare provider.
Athletes should never hide symptoms in an attempt to remain in a game or return to competition sooner.
Final Authority
L4 Lacrosse LLC reserves the right to withhold an athlete from participation whenever there is a reasonable concern regarding the athlete's safety.
Medical clearance establishes an athlete's medical eligibility to return; however, L4 Lacrosse may still withhold participation when circumstances reasonably indicate that additional evaluation or documentation is necessary.
No game is more important than an athlete's health. When in doubt, sit them out.
This policy is intended as an athlete-safety policy and does not replace evaluation, diagnosis, treatment, or return-to-play instructions from a qualified healthcare professional.
BLL Heat & Safety Policy
BLL Heat and Athlete Safety Policy
The safety of our athletes, coaches, officials, and families is the Bluegrass Lacrosse League’s highest priority. Because temperature, humidity, direct sunlight, and artificial turf can significantly increase heat stress, BLL will not make decisions based solely on the forecasted air temperature.
BLL will monitor conditions at the playing surface using an onsite Wet Bulb Globe Temperature (WBGT) or heat-index device, specifically the BT-881W Heat Stress Meter. Readings will be taken before play and monitored throughout the evening. The league may add cooling breaks, shorten games, delay play, modify the schedule, or cancel games whenever conditions become unsafe.
Heat Guidelines

Heat Index Below 95°F or WBGT 86.9°F and Below
-
Games may proceed as scheduled.
-
Water will remain available to athletes at all times.
-
Coaches will monitor athletes closely for signs of heat illness.
-
Players may request water or remove themselves from play at any time.
Heat Index 95–99°F or WBGT 87.0–89.9°F
-
Mandatory shaded water and cooling breaks will occur at least every 30 minutes.
-
Players must remove helmets and gloves while on the sideline.
-
Games may be shortened or played with a running clock.
-
Conditions will be rechecked at least every 30 minutes.
Heat Index 100–104°F or WBGT 90.0–91.9°F
-
Boys & Girls lacrosse activity will be suspended because helmets and protective equipment are required.
-
Play may not resume until onsite conditions return to an acceptable level.
-
Other activities may also be delayed, modified, or suspended at the league’s discretion.
Heat Index Above 104°F or WBGT 92°F and Above
-
All outdoor BLL activity will stop.
-
Games will be delayed, rescheduled, or canceled.
Athlete Preparation
-
Families should make sure athletes:
-
Begin hydrating well before arriving at the field.
-
Bring a large, clearly labeled water bottle.
-
Eat appropriately before competition.
-
Notify a coach if they have recently been sick, experienced vomiting or diarrhea, or have a history of heat illness.
-
Immediately report headache, dizziness, nausea, cramps, unusual weakness, chills, confusion, or difficulty walking.
No athlete will be penalized for requesting water, taking a break, or removing themselves from play because they do not feel well.
Emergency Response
Any athlete showing signs of heat illness will be removed from activity immediately. Confusion, unusual behavior, collapse, seizure, repeated vomiting, or loss of consciousness will be treated as a possible exertional heat-stroke emergency. Emergency medical services will be contacted, protective equipment will be removed, and rapid cooling will begin immediately.
BLL’s protocol is based on heat-safety guidance established by the Kentucky High School Athletic Association and recognized sports-medicine organizations. The league reserves the right to take additional precautions whenever field conditions, athlete behavior, or medical guidance indicate that play should be modified or stopped.
Schedule changes or cancellations will be communicated to families as quickly as possible through the league’s normal communication channels.
